How to Prepare for Your First Consultation

Before the first appointment with Simple Steps Nutrition, you will be sent a pre-assessment form to complete for either yourself or your child. It will contain information about our Privacy and Consent Policies and asks for information and consent from you. We request that this be completed 48 hours prior to your appointment. During an initial assessment we will ask what your expectations are and what you would like to gain from the appointment. We will ask a range of questions including medical history and investigations, medications, nutritional supplements, gastrointestinal health, blood tests, social history, daily routine, work and family commitments. A comprehensive dietary assessment will be completed. The Dietitian will set goals with your input and discuss simple strategies to implement a plan that will suit you and your family. It is helpful to bring any clinical information along to your appointment such as recent blood tests, medical tests, referrals or reports.

Cancellation Policy

We understand that life is busy and commitments can change. We require 24 hours notice to reschedule your appointment, or the full fee will apply. Please contact us directly to change your appointment.
